Why do I have to submit this form?

You placed your order over the telephone, so our system was not able to present the above mentioned legal documents to you. This form is to make sure that we are all on the same page regarding how our service works and what SingleHop's policies are. This form must be submitted before we will setup your servers. It only needs to be submitted once per client
